Plasmalogens, myelin and the ProdromeScan
Every nerve in your body is a wire, and every wire needs insulation. The insulation is built from a fat called a plasmalogen. When plasmalogens run low, the signal degrades: brain fog, fatigue, slower thinking, unsteady nerves. This is one of the few things in neurology we can measure early and do something about.
What a plasmalogen is
Plasmalogens are a special class of phospholipid, the fats that make up cell membranes. They are concentrated where signaling and energy matter most: the brain, the heart, the liver, the immune system and the mitochondria. They are made in the peroxisome, a small organelle that also handles detoxification and lipid processing, which is why peroxisomal stress and plasmalogen decline tend to travel together.
Inside a membrane, a plasmalogen does two jobs. It gives the membrane the right flexibility for receptors and transporters to work, and it acts as a sacrificial antioxidant: its vinyl-ether bond takes the oxidative hit so the rest of the membrane does not. Membranes with enough plasmalogen are resilient. Membranes without it oxidize, stiffen and leak.
Why myelin depends on them
Think of the nervous system as a high-speed network. Axons are the cables carrying signals between brain, spinal cord and body; myelin is the insulation wrapped around them that keeps the signal fast and on track. Run a signal down a bare wire and it weakens and scatters. Myelin is not a generic fat wrap; it is a specialized structure built largely from plasmalogens, produced by oligodendrocytes in the brain and Schwann cells everywhere else. Those cells need a steady supply of plasmalogen precursors to build, maintain and repair the sheath.
What drives them down
Three things, and most chronic illness has all three. Inflammation: autoimmune disease, persistent infection, metabolic syndrome, mold and biotoxin illness all inflame nervous tissue and erode myelin. Oxidative stress: the lipids in myelin are the first to oxidize when antioxidant capacity is exhausted. Aging: plasmalogen production falls with age, so repair slows exactly when demand rises. Hormonal transitions add to it; estrogen decline is associated with lower plasmalogen levels, which is part of why menopause so often arrives with brain fog and fatigue.
Low plasmalogens have been correlated with neuroinflammation and early neurodegeneration, with mitochondrial and peroxisomal dysfunction, and with the fatigue and cognitive symptoms that sit in the middle of so many "normal labs" cases. In Dr. Dayan Goodenowe's model (he developed the test), plasmalogen decline is not a downstream symptom but a core biochemical failure that other problems build on.
Who should consider testing
- Brain fog, slowed thinking or memory changes at any age
- Fatigue that does not match the standard labs
- A family history of dementia or neurodegenerative disease
- Perimenopause and menopause with cognitive symptoms
- Numbness, tingling, balance or nerve symptoms
- Chronic inflammatory illness: autoimmune, mold/CIRS, long COVID
- Anyone building a serious healthy-aging plan for the brain
The test
ProdromeScan quantifies more than 30 membrane lipids and metabolites from one blood draw: plasmalogens and their precursors, sphingomyelins and ceramides (membrane rigidity and stress load), cardiolipins (mitochondrial membranes), phosphatidylcholines, cholesterol and sterol intermediates (liver and hormone precursor balance), the DHA/EPA plasmalogen ratios that set neuroprotective versus inflammatory tone, and oxidative stress markers. It detects peroxisomal dysfunction years before cognitive or metabolic decline is obvious.
Conditions linked to low plasmalogens
Low plasmalogen levels are not a diagnosis; they are a measurable biochemical state that keeps showing up underneath a long list of conditions. These are the associations with the strongest published evidence, much of it from Dr. Goodenowe's own group.
Alzheimer's disease and dementia
Blood and brain plasmalogens are lower in Alzheimer's, track with how severe the cognitive decline is, and begin falling years before symptoms. In the 1,500-person ADNI cohort, plasmalogen indices related to diagnosis, cognition and CSF tau; post-mortem analysis ties low cortical plasmalogens to plaques, tangles and lower cognition.1-3
APOE4 carriers
In a community sample of 1,205 older adults, plasmalogen level and APOE genotype were independent predictors of cognition and dementia, and higher plasmalogens appeared to offset the added risk that comes with the ε4 allele. If you carry APOE4, this is the one modifiable lipid worth knowing.4,5
Cognition and mobility in aging
Higher plasmalogen biosynthesis is associated with better global cognition in older adults, and in a clinical trial of cognitively impaired persons, oral plasmalogen precursors raised blood levels, lowered oxidative stress markers and improved cognition and mobility.6,7
Parkinson's disease
Plasmalogens are reduced in the brain and blood of people with Parkinson's. In animal models, restoring them protected dopamine neurons and reduced levodopa-induced dyskinesias, the movement side effect of long-term treatment.8,9
Bipolar disorder and schizophrenia
Ethanolamine plasmalogens are altered in the blood of patients with bipolar disorder, and ether-lipid changes are documented in schizophrenia, consistent with the mitochondrial and endoplasmic-reticulum stress seen in both.10,11
Breast cancer
Serum plasmalogen deficiency, paired with overactive fatty-acid elongation (ELOVL5), distinguished breast cancer patients from controls in two case-control studies and shifted after surgery, pointing to a membrane-lipid signature of the disease.12
Myelin, nerves, vision and fertility
Animals that cannot make ether lipids develop defects in central nervous system myelination, cataracts and arrested sperm production; the inherited human peroxisomal disorders (Zellweger spectrum, rhizomelic chondrodysplasia punctata) show what complete deficiency does to the brain, bones and eyes.13
Chronic inflammation and oxidative stress
Because the plasmalogen vinyl-ether bond is sacrificed to oxidative stress, any sustained inflammatory state (autoimmune disease, persistent infection, mold and biotoxin illness, metabolic syndrome) consumes plasmalogens faster than the peroxisome can replace them. This is the mechanism behind the fatigue and brain fog that accompany those conditions.14

What we do with the result
A low plasmalogen result is one of the more satisfying findings in this practice, because the fix is direct. Plasmalogen precursors can be taken orally and the body finishes the synthesis; omega-3 precursors support brain and retina, omega-9 precursors support the myelin-building cells. Around that core we rebuild the rest of the membrane system: phosphatidylcholine (orally or as an IV series when the gut is not absorbing well), omega-3 balance guided by the DHA/EPA ratios, and the peroxisome and mitochondrial cofactors that let the cell use what you give it (riboflavin, niacin, CoQ10, carnitine, methylation support). The scan is repeated to confirm the numbers moved, not just the symptoms.
The same lipid panel explains things that look unrelated: a "dysfunctional" HDL, an unexplained ALT, a small-dense LDL pattern, or a ceiling on how much a mitochondrial peptide can do. Membranes are the terrain everything else acts on.
How to get it
ProdromeScan is ordered through the practice and interpreted at a Results Review visit, where it is read alongside your other labs. Plasmalogen precursor products are available directly from Prodrome; our code takes a percentage off.
Plainly
Plasmalogen supplements support membrane and myelin structure; they are not a treatment for any disease, and the statements here have not been evaluated by the FDA. Neurological symptoms need a proper evaluation. This article is education, not a plan for you.
